Koru Hospital Breast Health Center is a comprehensive center offering a wide range of services, from early diagnosis and surgical treatment of breast diseases to screening programs and rehabilitation. We implement protocols in accordance with national and international guidelines through a multidisciplinary team approach in breast cancer screening, diagnosis, and treatment processes.

The Importance of Early Diagnosis in Breast Cancer

Breast cancer is the most common type of cancer in women worldwide and is also the leading cancer among women in Turkey. While the 5-year survival rate for breast cancer caught at an early stage is over 95%, this rate drops significantly in advanced stages. Therefore, regular screening programs and conscious self-examination are of life-saving importance.

Screening and Imaging Methods

Mammography

Digital mammography is the gold standard imaging method for breast cancer screening. The latest generation digital mammography devices used in our center obtain high-resolution images with a low radiation dose. Screening recommendations:

  • Annual mammography for women over 40 years old
  • Screening with mammography and MRI starting from age 30 for the high-risk group (family history, genetic predisposition)
  • Annual MRI screening starting from age 25 for BRCA1/BRCA2 mutation carriers

Breast Ultrasonography

Breast ultrasound is the most important imaging method that complements mammography. It has high diagnostic value, especially in young women with dense breast tissue, in cases where mammography is insufficient. Ultrasonography is an indispensable tool in distinguishing cystic and solid lesions, evaluating axillary lymph nodes, and biopsy guidance.

Breast MRI

Contrast-enhanced breast MRI is used for screening in high-risk women, for further evaluation of suspicious lesions detected on mammography and ultrasound, and for monitoring the response to neoadjuvant chemotherapy. Breast MRI reveals the size and extent of lesions most accurately with the advantage of soft tissue contrast and multi-planar imaging.

Tomosynthesis (3D Mammography)

Digital breast tomosynthesis allows for three-dimensional imaging of breast tissue in thin sections. It increases the detection rate of small lesions and reduces unnecessary recall rates by eliminating the problem of tissue overlapping, especially in dense breast tissue.

Biopsy and Pathological Evaluation

Tissue sampling is required to confirm the diagnosis in lesions found to be suspicious on imaging. Biopsy methods applied in our center:

  • Fine Needle Aspiration Biopsy (FNAB): For cystic lesions and cytological evaluation
  • Core Needle Biopsy (Tru-cut): The standard method performed under ultrasound or mammography guidance, providing histopathological diagnosis
  • Vacuum-Assisted Biopsy: For more tissue sampling in microcalcifications and small lesions
  • Stereotactic Biopsy: For lesions visible only on mammography and not detectable on ultrasound
  • MRI-Guided Biopsy: For lesions detected only on MRI

The tissue samples obtained are evaluated via immunohistochemical analysis for estrogen receptor (ER), progesterone receptor (PR), HER2 status, and Ki-67 proliferation index. These parameters play a decisive role in treatment planning.

Surgical Treatment of Breast Cancer

Breast-Conserving Surgery (Lumpectomy)

Breast-conserving surgery aims to preserve the breast by removing the tumor with adequate surgical margins. This approach, which offers survival results equivalent to mastectomy in early-stage breast cancer, is applied in conjunction with radiotherapy. By using oncoplastic surgery techniques in our center, both complete tumor removal and cosmetically satisfactory results are achieved.

Mastectomy

Mastectomy is performed in cases where the entire breast needs to be removed due to tumor size, multifocality, or patient preference. Types of mastectomy performed in our center:

  • Skin-sparing mastectomy: Allows for simultaneous reconstruction by preserving the breast skin
  • Nipple-sparing mastectomy: Preservation of the areola-nipple complex in suitable cases
  • Modified radical mastectomy: Removal of breast tissue and axillary lymph nodes together

Sentinel Lymph Node Biopsy

Sentinel lymph node biopsy (SLNB) is a standard approach that prevents unnecessary axillary dissection in clinically node-negative patients. The first draining lymph node, marked with radioisotope and/or blue dye method, is removed and sent for frozen and permanent pathological examination.

Oncological Treatment Coordination

Oncological treatments applied after or before (neoadjuvant) surgical treatment are planned by the decision of the tumor board (multidisciplinary council):

  • Chemotherapy: Adjuvant or neoadjuvant regimens (anthracycline, taxane-based protocols)
  • Radiotherapy: Standard after breast-conserving surgery, in selected cases after mastectomy
  • Hormone therapy: Tamoxifen or aromatase inhibitors in ER/PR positive tumors
  • Targeted therapy: Trastuzumab and pertuzumab in HER2 positive tumors
  • Immunotherapy: In selected patients with triple-negative breast cancer

Breast Reconstruction

Breast reconstruction after mastectomy is of great importance in terms of patients' perception of body integrity and quality of life. Methods applied by our plastic and reconstructive surgery specialists in our center:

  • Implant-based reconstruction: Tissue expander followed by permanent implant or single-stage direct implant
  • Autologous tissue reconstruction: DIEP flap, TRAM flap, latissimus dorsi flap
  • Contralateral breast symmetrization: Reduction or lift of the opposite breast when necessary
  • Nipple-areola reconstruction: Creation of the nipple and areola in the final stage

Benign Breast Diseases

In addition to breast cancer, our center provides services for the diagnosis and treatment of benign breast diseases:

  • Fibroadenoma: The most common benign breast tumor in young women, follow-up or excisional biopsy
  • Fibrocystic changes: A common condition characterized by breast pain and nodularity
  • Breast cysts: Aspiration for simple cysts, further evaluation for complex cysts
  • Intraductal papilloma: A common cause of bloody nipple discharge, ductal excision
  • Mastitis and breast abscess: Infections during breastfeeding or non-puerperal infections
  • Gynecomastia: Breast enlargement in men, medical and surgical treatment

Genetic Counseling and Risk Assessment

Genetic risk assessment is of great importance in individuals with a family history of breast and/or ovarian cancer. Services offered in our center:

  • BRCA1/BRCA2 and other high-penetrance gene mutation tests
  • Multigene panel tests: Evaluation of genes such as PALB2, ATM, CHEK2, TP53
  • Discussion of risk-reducing surgical options (prophylactic mastectomy, oophorectomy)
  • Creation of a personalized screening program for high-risk individuals
