For patients presenting with dizziness, the objective evaluation of the balance system in the inner ear is of great importance for an accurate diagnosis. One of the methods accepted as the gold standard for this evaluation is the caloric test. The caloric test is one of the most important parts of the vestibular assessment battery, measuring the functional performance of the horizontal semicircular canal in the inner ear separately. As it is one of the rare tests capable of detecting unilateral balance loss, it plays a decisive role in the clinical decision-making process.
What is a Caloric Test?
The caloric test is a balance test based on the principle of stimulating the balance system in the inner ear by delivering water or air at a controlled temperature into the external ear canal and recording the resulting eye movements (nystagmus). Since the right and left inner ears are stimulated separately, the functional strength of both sides can be evaluated comparatively. In this respect, the caloric test is considered the most reliable examination for revealing unilateral weaknesses of the balance system. The test has been used for decades to evaluate the branch functions of the balance system and its sensitivity has been brought to the highest levels today by combining it with digital videonystagmography systems.
Working Principle
The warm or cold stimulus applied to the area near the eardrum causes the movement of the fluid (endolymph) in the inner ear. This movement stimulates the sensors in the semicircular canal and sends a signal to the brain. In response to this signal, the brain creates specific movements in the eyes. By analyzing the direction, speed, and duration of these eye movements recorded with a videonystagmography (VNG) device, the right and left inner ear functions are measured numerically. The responses generated by cold and warm stimuli should normally be symmetrical; in cases where asymmetry is detected, inner ear disease is considered.
In Which Diseases Is It Used?
The caloric test is used as an auxiliary diagnostic method, especially in the following clinical conditions:
- Vestibular neuritis: Unilateral inflammation of the balance nerve
- Meniere's disease: Fluid pressure disorder in the inner ear
- Acoustic neuroma: Benign tumors originating from the balance nerve
- Labyrinthitis: Inflammatory diseases of the inner ear
- Traumatic inner ear injuries: Balance disorders following head trauma
- Drug-induced toxic effects: Damage to the inner ear by ototoxic drugs
- Unexplained chronic balance problems: Age-related vestibular losses
- Dizziness developing with sudden hearing loss
- Recurrent vertigo attacks
- Follow-up before and after vestibular rehabilitation

The caloric test offers many unique advantages in vestibular assessment:

How Does the Treatment Process Proceed?
Before the test, the patient's history of dizziness is taken in detail. The nature, duration, accompanying symptoms, and previous treatments of the complaint are noted. During the ear-nose-throat examination, the external ear canal and eardrum are checked; if there is an active infection or eardrum perforation, the test is postponed. After the hearing test and other vestibular tests are completed if necessary, the patient is taken to the room where the caloric test will be performed.
Test Application
The patient is placed in a supine position with the head elevated at 30 degrees, and VNG goggles that record eye movements are worn. First, a cold stimulus is applied to one ear, followed by a warm stimulus; the same procedure is repeated for the other ear. A rest period of 5-10 minutes is given between each stimulus. The total test duration is approximately 30-40 minutes. The patient may feel short-term dizziness; this is an expected response and is temporary. Dizziness that occurs after the test usually passes completely within half an hour.
Interpretation of Results
The maximum speed of the recorded eye movements is compared for both ears. Asymmetry above 25 percent is considered pathological and is evaluated by the physician to create a treatment plan. When the results are interpreted together with other vestibular test findings, the clinical diagnosis becomes clear.
Points to Consider
Some precautions should be taken for the accuracy of the test results:
- Sedatives and antihistamines should not be used for 48 hours before the test.
- The test is not performed if there is an active ear infection or a perforated eardrum.
- Patients should be fasting for the last 4 hours before the test.
- Eye makeup should not be worn.
- Alcohol and caffeine should not be consumed before the test day.
- Vehicles should not be driven until dizziness passes after the test.
- Regularly used medications must be reported to the physician.
- The presence of cerumen (earwax) should be cleaned before the test.
- If possible, a return home accompanied by an attendant should be planned after the test.
Comparison of the Caloric Test with Other Vestibular Tests
Although the caloric test is an important part of the vestibular assessment battery, it is not sufficient on its own. In modern vestibular assessment, other tests such as vHIT (video head impulse test), VEMP (vestibular evoked myogenic potential), and posturography are used along with the caloric test. Each test evaluates a different structure of the balance system; the caloric test specifically measures the response of the horizontal semicircular canal to low-frequency stimuli. Therefore, performing vHIT for high-frequency stimuli and VEMP tests for otolith organs as complementary tests significantly increases diagnostic accuracy.
Caloric Test in Pediatric Patients
Performing a caloric test in pediatric patients requires special experience and patience. Detailed information is provided before the procedure, and parental support is ensured so that the child does not get scared and can remain still during the test. In children, the caloric test provides important information, especially in evaluations after congenital balance disorders, ototoxic drug exposure, and head trauma.
